  to parents of children with Down syndrome. Facilitators are Anne Mancini 
 and Leah Janke.\n\nThe purpose of the training is to provide parents with 
 the skills\, knowledge\, and resources they need to coordinate care for ch
 ildren with genetic condition in partnership with a medical home.*\n\nBy t
 he end of the training participants will:\n\nIdentify the components of a 
 Medical Home\nDefine their role in coordinating care for their child\nIden
 tify one technique they can use to build a health partnership\nUnderstand 
 basic guidelines for advocacy\nRecognize effective communication skills\nI
 dentify key members of the child’s care team\nExpand concept of care coord
 ination beyond medical\nKnow how to use Coordinated Care Plan to facilitat
 e communication between care providers \nUnderstand the need to plan for c
 hild’s transition\nIdentify two different methods of organizing informatio
 n\nIdentify one resource that can help them navigate insurance\nIdentify o
 ne technique for dealing with denied insurance claims\nIdentify three key 
 concepts for evaluating resources\n\n*The goal of a medical home is to pro
 vide a rationally organized health care system that ensures that patients 
 who can benefit from specialty care gain timely access.\n\nApplications fo
 r $100 stipends for childcare are available. See registration form.
